TO REGISTER:
1. Complete a registration form. (You'll need the free Acrobat Reader to open, view and print the form. If you have any difficulty with this step, see "Problems" below.)
Choose one of two forms: Either choose the form you print off first, then fill in manually, or the one that allows you to keyboard in the info -- it calculates the totals for you -- and then you print it.

Note the "postmarked on or before" date for earlybird pricing - the date's on the form.
2. Write a check or money order payable to FOOTMAD.
3. Mail completed form with your payment to:
